TREATMENTS - Terms of service.

I declare that I am over 18 years of age (or have otherwise provided parental consent) and acknowledge and understand that I have voluntarily chosen to participate in this treatment offered by ALONJÉ or Partners operating within the Alonjé premise. I understand the nature of the treatment with ALONJÉ and am qualified, willing and able to participate as such.​

I agree that prior to my treatment I will inform ALONJÉ of any known medical conditions (including but not limited to pregnancy) or factors that may place, or the practitioner at risk. ALONJÉ may request a medical release from my medical practitioner prior to participation. I will inform ALONJÉ of any symptoms before, during and after participation in a ALONJÉ class . I also understand that if I am a prenatal or postnatal client, that I must consult with my physician and receive clearance prior to the treatment.

If I experience any pain or discomfort during the session, I will immediately inform the therapist so that the pressure and/or strokes may be adjusted to my level of comfort. I further understand that Massage Therapy should not be construed as a substitute for medical examination, diagnosis, or treatment and that I should see a physician, chiropractor, or other qualified medical specialist for any mental or physical ailment of which I am aware. Because Massage should not be performed under certain medical conditions, I affirm that I have stated all my known medical conditions, and answered all questions honestly. I agree to keep the therapist updated as to any changes in my medical profile and understand that there shall be no liability on the therapist’s part should I fail to do so. ​

I acknowledge and give consent to ALONJÉ and staff that as part of the treatment hands, arms may be used. Further more I agree to removing clothing for the treatment, with draping offered over breasts. I acknowledge that underwear must be worn at all times, and that I conduct myself appropriately and with respect towards ALONJÉ staff fitting with the treatment. I hold the right to decline this service, and must communicate this to ALONJÉ prior to treatment. This is a Therapeutic Massage session and any sexual remarks or advances will terminate the session and I will be liable for payment of the scheduled treatment. Therapist practitioner reserves the right to refuse services to me for any reason that they it deem necessary.

I acknowledge that any lateness due on my part will result in the treatment being shortened to accommodate my original booking time and out of respect to other clients following me. In this case, regardless of the length of the treatment I am responsible to pay for the service in full.

I fully accept and assume all such risks associated with ALONJÉ and release ALONJÉ and its staff from any and all liability for any loss, damage, injury or expense that I may suffer, or that my next of kin may suffer as a result of my participation in the classes, activities and services provided by ALONJÉ . I agree to hold harmless and indemnify ALONJÉ and its employees and agents from any and all liability for any damage to the property of, or personal injury to, any third party, resulting from my participation in any program, activity or service provided by ALONJÉ If any portion of this agreement is held invalid, I agree that the remainder of the agreement shall remain in full force and effect.

I declare that I have advised ALONJÉ of any injury, back, neck or joint pain, restricted movement, heart issues, asthma, or high or low blood pressure, arthritis, slipped or bulging vertebral disk, pelvic floor conditions, dizziness, diabetes, epilepsy, hernia, bone degeneration, high cholesterol, allergies or chronic illness. I also declare that I have notified ALONJÉ if I am pregnant and/or have given birth in the last 6 months, or if I have undergone surgery in the past 12 months.

ALONJÉ shall not undertake any obligation (whether contractually, at common law or otherwise) to advise or treat me in relation to any of the matters referred to in the preceding paragraph. I acknowledge that it is my obligation and mine alone to take responsibility for my health and wellbeing during any type of exercise I undertake with ALONJÉ . ​

I acknowledge that ALONJÉ shall not be liable or responsible to me for articles lost, damaged or stolen from any of its venues.

I waive the right to use, or teach intellectual property shared/ shown or communicated by ALONJÉ including but not exclusive to treatment combinations, coaching practices, courses, written material, photographic or video material from ALONJÉ, without the specified written request from the user to ALONJÉ and confirmation from ALONJÉ to the user.

Any persons entering and/or using our Rejuvenation space does so at their own risk and management. Alonjé is not responsible or the well being of persons using the space and or hot cold therapies. We do not recommend this space for anyone with underlying medical conditions or pregnancy.

The terms and conditions of this release form and waiver of liability are subject to change without notice.

All persons under 18 years of age must have this form signed on their behalf by a parent or guardian before attending a ALONJÉ class. Once the parent or guardian has signed the waiver, persons under 18 years of age may ALONJÉ .
